Job Description:
To assess, evaluate, plan, provide, and coordinate a plan of treatment under the orders of a physician for home health patients as a Skilled Nurse, also taking the rose as case manager.
Provide a variety of health care services centered around the unique and individual needs of each patient according to the plan of care; meet the needs of all customers including patients, significant others, physicians, and other staff members of Hammond-Henry Hospital; work in multidisciplinary teams in order to deliver a full range of services and participate in team based continuous improvement.
The Registered Nurse shares the responsibility of 24 hour on call.
They are responsible for supervising the Certified Nurse Assistant.
Access to information is limited to minimum necessary to perform the duties of the job.
The supervisor assists the manager in the role specific to meet home health legal, medical, and staff guidelines.
Job Requirements:
Valid nursing licensure by the State of Illinois Department of Professional Regulation in good standing.
Current BLS certification required.
MUST have reliable transportation.
